Renew annual mobile food facilities permit and request inspection
Annual permit fees are due prior to the permit expiration date, and payment alone does not grant the right to operate. We will issue a permit after you have paid the applicable fees and your facility passes its annual inspection.
Required documentation
To renew your annual permit, you must complete the following application documents:
- A completed and signed MFF renewal cover sheet.
- A completed and signed Commissary Agreement unless you operate a produce vehicle.
- If overnight parking is not at a food facility, complete MFF storage agreement. Only CMFOs may be stored at a private home upon completing CMFO self-certification checklist and approval.
Any changes from the previous year?
If there any changes from the previous year to any item on this list, we require you submit a related document to reflect the changes:
- Menu, including the ingredients
- Standard operating procedures (SOPs)
- A copy of the DMV registration, such as changes to license plate.
- MFF route sheet
Submit application, pay and complete inspection
- Gather the paperwork listed above and submit them by email or in-person at our office between 730am to 415pm.
- Pay your annual operating permit renewal fee online or in-person at our office.

3. Once your complete renewal application is reviewed, approved and fee paid, we will contact you to schedule an inspection. If you are more than 15 minutes late to your scheduled inspection, you may be asked to reschedule. Please be prepared to wait in the event the inspection prior to your appointment runs longer than expected. Inspections are subject to change.
